Life expectancy over the past decades has continued to increase. However, prostate cancer is a disease that occurs more frequently in later life. Since the aging population is experiencing an enhanced health status, every effort should be devoted to providing curative measures in elderly patients. The focus of this study is to evaluate clinical, functional and oncological, outcomes in men 70 years of age and older undergoing robot-assisted radical prostatectomy (RARP). METHODS From January 2008 to September 2011 3,422 consecutive patients underwent RARP for localized prostate cancer by a single surgeon (V.P.). Patients aged 70 years and over (N=400) were selected for this study. There were 250 patients with at least one year of complete follow-up which constitutes the clinical material for the study. The severity of complications was defined according to the Clavien-Dindo classification (Grade I-V). RESULTS The mean age was 72.1 years (SD ± 2.3). The D'Amico risk classification identified 116 patients (46.4%) as low risk, 96 patients (38.4%) intermediate risk and 38 patients (15.2%) high risk. The mean operative time was 80.0 ± 18.3 minutes. Estimated blood loss (cc) was 120.0 ± 49.8. No patient required transfusion or open conversion. There were no intraoperative complications and 232 patients (92.8%) experienced no postoperative complications. Grade I complications were reported in 18 patients (7.2%) and Grade IIIb in 1 patient (0.4%). The most common clinical stage was pT2c in 176 patients (70.4%). Positive surgical margins were present in 36 patients (14.4%). Mean hospital length of stay was 1.3 ± 0.9 days. At 12 months followup, freedom from biochemical recurrence was 94.0%, continence 86.0% and potency 55.8%. CONCLUSIONS This study clearly demonstrates that RARP can be performed in elderly patients without any increase in perioperative morbidity, excellent oncologic and continence outcomes as well as acceptable potency rates.
