About
Tom McNicholas is a leading urological surgeon and researcher whose work has significantly advanced the diagnosis and surgical treatment of prostate cancer. His research centers on improving preoperative risk stratification and refining the learning curve for robotic-assisted surgery. McNicholas was instrumental in validating the Prostate Health Index (PHI) and its derivatives, %p2PSA, as powerful predictors of adverse pathology in men undergoing radical prostatectomy. This multicentric European study, cited over 88 times, demonstrated that these novel biomarkers outperform traditional PSA in identifying aggressive disease, directly informing clinical decision-making. Beyond diagnostics, he has pioneered the systematic analysis of surgical learning curves for robot-assisted laparoscopic prostatectomy. His landmark studies, including a real-time assessment of over 1,000 cases, established objective benchmarks for surgical proficiency and helped define the structure of robotic fellowship programmes. Notably, his work on the learning curves of two surgeons from a single UK centre provided critical evidence for safe, phased implementation of robotic services. McNicholas has also investigated the clinical significance of anterior prostatic fat, challenging conventional lymph node dissection templates.
