Subxiphoid Single-Port Robotic-Assisted Thoracoscopic Surgery Using da Vinci SP: Early Experiences

Abstract
The da Vinci single-port (SP) system is the latest advancement in robotic surgery, designed to perform complex procedures through a single incision.This system offers improved dexterity, enhanced visualization, and reduced surgical trauma compared to conventional multiport robotic systems.While the da Vinci SP system has shown promising outcomes in urology, general surgery, and gynecology, its application in thoracic surgery is still emerging.This presentation aims to report our initial experiences with the da Vinci SP system via the subxiphoid approach in patients who underwent thymectomy for anterior mediastinal masses.Between October 2020 and April 2025, a total of 9 patients were included in this study.The mean age was 54.69.9 years, and 4 patients (44.4%) were male.The mean body mass index was 24.53.5 kg/m 2 , and 3 patients had myasthenia gravis.Complete resection was achieved in all cases without conversion to multiport or open surgery.The mean operation time was 136.949.8minutes.Seven patients (77.8%) were diagnosed with thymoma, with a mean tumor size of 33.816.3mm.The mean peak Numeric Rating Scale score was 3.81.4.The mean durations of chest drainage and hospital stay were 1.80.7 days and 2.80.8days, respectively.No postoperative complications were observed.The da Vinci SP system enables safe and feasible resection for thymectomy with minimal surgical trauma and rapid recovery.With its potential for improved patient outcomes, further clinical experience and technological advancements are essential for expanding its indications in general thoracic surgery.Session 3
