Uterine atony is a condition in which the uterus fails to contract properly after childbirth, preventing the normal compression of blood vessels at the placental attachment site. Under typical postpartum physiology, uterine muscle contractions clamp down on these vessels to control bleeding; when the uterine muscle remains flaccid or poorly toned, uncontrolled hemorrhage can result. It is the leading cause of postpartum hemorrhage worldwide, accounting for the majority of severe maternal bleeding events. Management ranges from uterotonic medications and uterine massage to more invasive interventions such as balloon tamponade, surgical ligation of uterine vessels, or, in refractory cases, peripartum hysterectomy. In the context of robotics and minimally invasive surgery, uterine atony is clinically relevant because it can necessitate robotic-assisted laparoscopic hysterectomy, particularly when combined with complications such as abnormal placentation. Understanding uterine atony is important for engineers and researchers developing surgical robotic systems, hemorrhage-detection algorithms, and automated decision-support tools aimed at improving maternal outcomes in high-risk obstetric scenarios.
